This report is an evaluation of four after-care initiatives under the after-care package launched by the Danish National Board of Social Services in connection with the 2011-2014 public pool of funds agreement.
The overall purpose of the evaluation is to examine whether the four after-care initiatives help support previously placed young people between 18 and 22 years in their transition to adult life. The four initiatives are:
'The road to education and employment', which is a municipal collaboration model that paves the way for new cross-disciplinary organisation for professional collaboration.
'Systematic support programmes from voluntary organisations for previously placed young people', in which voluntary organisations collaborate with municipalities on providing support for previously placed young people.
'Trials with host and contact families', which enable previously placed young people to live with a host family or be connected with a contact family who can support the young people in their transition from placement to living on their own.
'Trials with social entrepreneurship', in which institutional care homes collaborate with local enterprises to create internships for young people in placement.
